New Employees/Volunteer

  1. The department Completes Part 1 and faxes the attached Scheduling/Notification Form to Workforce Medical Clinic at 650-568-9053.
  2. Workforce Medical schedules an appointment with the employee/volunteer (by phone) and notifies Risk Management of the date and time of the appointment. (Part 2 of the Scheduling/Notification form)
  3. After completion of the examination, Workforce Medical notifies the department if the individual is cleared to hire (part 3 of the Scheduling/Notification Form). The department is notified by Risk Management if any box in Part 4 is checked. Risk Management works with the individual and an occupational health consultant (Richard Cohen, MD) to attempt to resolve any restrictions or pending cases so that employee/volunteer can be hired/released for duty.

Current Employees/Volunteers

  1. Annually or as designated (prior to the anniversary of their last examination) Risk Management completes Part 1 and faxes to Workforce Medical the attached Scheduling/Notification Form.
  2. Workforce Medical schedules an appointment with the employee/volunteer (by phone) and notifies Risk Management of the de and time of the appointment. (Part 2 of the Scheduling/Notification form)
  3. After completion of the examination, Workforce Medical notifies the department if the individual is cleared to hire (part 3 of the Scheduling/Notification Form). The department is notified by Risk Management if any box in Part 4 is checked. Risk Management works with the individual and an occupational health consultant (Richard Cohen, MD) to attempt to resolve any restrictions or pending cases so that employee/volunteer can be hired/released for duty.

Restricted Status for Either New or Current Employees/Volunteers

  1. When new or current employees receive a restricted status and Workforce Medical requires further evaluation or testing, it is the responsibility of the employee to get the recommended testing or follow-up. If the volunteer has health insurance, the County will reimburse the volunteer for their out-of-pocket expense.
  2. If the volunteer has no health insurance, the County will reimburse the volunteer for the entire amount of the test. If the employee/volunteer wants to appeal either the requirement for further testing or the results of the further testing, he/she should call Elaine Simmons at 650-363-4696. Elaine will coordinate a meeting or phone conversation between the employee/volunteer and Dr. Richard Cohen, the County's occupational health consultant. There is no cost for the appeal.
