What is the Kaiser Permanente Telephonic Wellness coaching Program?

Wellness coaching provides personalized support to help you develop a plan that fits your needs and lifestyle, for the kind of change you want to make: manage your weight, quit smoking, eat healthier, exercise more, or reduce your stress. Coaching takes place in a safe, supportive environment where you will recognize your past successes and identify obstacles that prevent you from making healthy choices now.

During the process, you will identify a behavior that you want to change, set goals, and break your goals into manageable steps. Kaiser Permanente coaches will provide support, guidance, and accountability along the way. Wellness coaching through Kaiser Permanente is connected with your overall health care. This means that your wellness coach, your physician, and your entire health care team can communicate to provide coordinated care.

To make an appointment, please call 1-866-251-4514, seven days a week, from 6 a.m. to midnight. Coaching sessions are available Monday through Thursday, from 8:30 a.m. to 7 p.m., and Friday from 8:30 a.m. to 5 p.m.

Who is eligible to participate in the Kaiser Permanente Wellness coaching Program?

The Program is offered to benefits-eligible County and Courts employees who belong to a County Kaiser Permanente health plan, either as a subscriber or dependent, and who are high risk for future heart problems and/or diabetes as determined by their clinical results at the Wellness Screenings held at the beginning of this year.

Prospective participants must be ready and willing to make at least one significant change in their lifestyle during their participation in the program. As a participant, you will be actively engaged in learning about your health and in making healthy changes. If you are not ready to do that, this program may not be for you.

If you have not been personally invited by the Wellness Coach, you may not be eligible because you are not considered “high risk”.

Am I required to participate in this program?

NO, the Kaiser Permanente Wellness coaching program is entirely voluntary and is only offered to those who meet certain high risk criteria. If you have not been personally invited, you may not be eligible because you are not considered “high risk”.

How much does it cost?

Wellness coaching is free of charge to those who participate.

How long is the program?

The Wellness coaching Program consists of four sessions with a Wellness Coach and must be completed within a period of five months.

I got an invitation to participate. How do I sign up?

Call our toll-free number, (866) 251-4514, to speak with a telephone service representative to schedule your first coaching appointment.

The final date to register for this year’s program is July 31, 2015.

How much time will it take?

The first session lasts up to 30 minutes.

Follow-up coaching sessions are usually 10 to 15 minutes and typically take place over several months.

All sessions are conducted by telephone and email.

What typically happens during a Telephonic Wellness coaching session?

All sessions provide:

  • A safe, supportive, and nonjudgmental conversation with a trained professional

  • Assistance in defining what kind of change you want to make

  • An assessment of your readiness to make a change

  • Assistance in setting achievable goals

  • Help with identifying and dealing with your specific barriers to achieving success

  • Encouragement and support along the way

  • Information about helpful Kaiser Permanente and community resources

  • Follow-up and accountability

During the first session you will:

  • Get to know your coach.

  • Decide which health topic you would like to focus on.

  • Assess your readiness to make a change in that area.

  • Choose a small goal or action step to begin.

You will talk to the same Wellness coach at each appointment. You can request a different Wellness coach at any time if you are not satisfied.

Are there any additional Wellness Rewards for participating?

YES! You are eligible to receive an additional $50 Wellness Reward for completing this program before November 15, 2015.

What do I have to do to get a Wellness Reward?

You must complete a minimum of four coaching sessions to qualify for $50 in Wellness Rewards. The four sessions must be completed within five months of your first session with the Wellness Coach. You are asked to set and then work on accomplishing several health goals over the course of your wellness coaching period that will reduce your health risks. Individual health improvement goals will be discussed at the first coaching session.

Who keeps track of my telephonic counseling sessions and determines my eligibility for Wellness Rewards?

Your Wellness coach will keep track of your counseling sessions. The Kaiser Permanente Worksite Wellness group will determine your eligibility for wellness rewards.

In December, you will receive an email from the Kaiser Permanente Worksite Wellness Group with a request to complete an online evaluation of the program. At that time, Kaiser Permanente will also confirm whether or not you completed the required four counseling sessions to be eligible for an additional $50 Wellness Reward.

When will I get my Wellness Reward?

You will get your Wellness Reward in March of the following year. It will be a $50 Target gift card and it will be sent to you directly by the Wellness coach.

Who provides the Wellness coaching?

Kaiser Permanente’s Wellness coaches include health educators who are lifestyle change experts, with specific expertise in health promotion, prevention, and behavior change counseling. Coaches are trained in motivational interviewing, a technique proven to be effective for guiding behavior change. Wellness coaches typically possess a Masters-level degree.

Through engagement, collaboration, and encouragement, coaches are able to build rapport and facilitate motivation toward healthier behaviors. Your coach will provide information and support based on your readiness to make a healthy change.

Your Wellness coach will also help you match your needs and preferences with the best resources to support your success. Wellness coaches speak English and Spanish. Interpreters are available for all other languages.

You will talk to the same Wellness coach at each appointment. You can request a different Wellness coach at any time if you are not satisfied.

How is my health information managed?

YOUR PARTICIPATION IN THIS PROGRAM IS CONFIDENTIAL. Your name is not shared with the County at any time. It is up to you to disclose who you want to know about your participation in this program. If you want to participate in this program on County time, you most likely will have to discuss this with your supervisor and let him/her know of the time that you will need to be away from work. That is why we always state that you have the option of participating in this program on your own time if you want it to be totally confidential.

YOUR HEALTH INFORMATION IS PROTECTED. When you participated in the Wellness Screening, you signed a release statement limiting who could view your health information. Your name is not shared with the County at any time, unless you authorize it.

The Kaiser Permanente Wellness Coach Program and Kaiser Permanente’s Worksite Wellness Group are responsible for compiling and providing aggregate participation information to the County. No personal health information is shared. Any analysis of health data is done in aggregate and when doing so, your name and any personal identifiers are removed from the health data.
